I have a TOYOTA RAV4 d-cat 2.2 2007 year. Engine is replaced in july 2010 by the dealer. Since that day, if the car hasn't been used for a couple of days the engine makes a loud noise when starting cold - only the first stroke is hard (loud). After that, the next start (ignition) sounds normal.

Sounds like oil is draining away from the oil filter.. failed non return valve?

Try changing the oil filter with a new Toyota one - ensuring the new one is filled with oil when fitted.

This probem is since july 2014 (replaced engine). My car is always maintained at Toyota dealer.

One Toyota mechanic told me (unofficially) it could be a faulty injector.

That sounds plausible, but scary. Any idea?
