Stuck Accelerator Pedal?

My Avensis is 2012 plate 2.0 diesel with 22k on the clock owned and driven by me from new; also serviced at Mr T from new. On 2 occasions recently, joining the motorway from the slip road, I've floored the accelerator in 3rd gear to safely merge with the traffic then dipped the clutch ready for 4th only for the engine to rev up to max as if the pedal was stuck to the floor. It happened again today and I'm assuming (hoping) that it is because the pedal was wedged under the (loose) rubber floor mat. It's all a bit unnerving.

Are there any known issues with the Avensis accelerator pedal sticking?

Not afaik. However, last time my car was in for service when I collected the car the dealer had placed my non-oem rubber mats in the boot & the visual safety report had something on it about potentially interfering with pedal operation.

If in doubt buy some OE Toyota mat fixings to secure the mats.
