Avensis Gearbox Issue Please Help!

Hi Guys

I have an Avensis V plate and im having some issues!!! I can still drive but i can only get 1st gear 2nd gear Reverse and 5th gear. For some reason i cant get 3rd and 4th gear i can put the gear stick into 3rd and 4th but i cant get any acceleration once its in the gear. I would be really happy if someone could tell me whats wrong with it and how i can get this fixed and if possible a rough price for the job. thanks guys and girls
