Vibration when accelerator pressed down

2006 avensis 2.4 diesel saloon.  As i put press accelerator down there is a vibration from the engine, whether moving or still, and it has gradually got worse.I had my turbo charger replaced a year ago and was told that something was adjusted to improve perfomance.  Then the vibration only kicked in at high revs.  Now I can be stationary, touch the accelerator slightly with the clutch under slight pressure and the vibes start, getting worse as the revs increase.  Any advice will be gratefully received as it is driving me mad.

Does the engine shake if you have clutch fully  depressed? 

Why do you hold clutch under slight pressure?

Thank you for your reply.  Thinking about it, it's more of a vibration than a shake.   No.  Tried it to rule out other possibilities,like a loose exhaust, which it did.
