Master cylinder push rod clevis

Hello everyone, really need some help please. 2006 Corolla 2.0 D4D.

Something has sheared off the clutch pedal assembly. I bought a pedal assembly from breakers but it doesn't have the clevis on it. I need to be able to reconnect the sub assembly to the push rod. Where can I find the correct clevis? Or correct size? Do I also need to buy the push rod? Or is there a way to fix a new clevis to this? Hope this makes sense! Thanks Craig
