2008 Rav4 limpmode

My 2008 rav4 2.2 diesel every so often goes into limpmode, it could take a few hours of driving before this happens and either by disconnecting the Battery or just turning it off and on it returns to normal, it has almost 400k up on it. I am wondering if it could be a blocked egr/inlet manifold or a blocked pickup in the diesel tank.

Have you read the fault codes when this happens?
