Townace diesel transmission problems

I have 1990 diesel townace. Just recently reverse gear is delayed into engaging, and when it does there is a clunk sound on drive line. Also it does seem it wants to shift regularly into the higher overdrive gears.  And when it does it wont stay there for very long, wants to shift back down causing the rpm to shoot right up. Any ideas on fixing the problem
