Having been around engines for a lifetime, I rather thing you should consider an internal cylinder head blowing, or cylinder gasket.

Hi folks, my name's Richard Hughes (Rick) I've been a Toyota driver now for 20yrs the first 14yrs in a Corolla 2.0ltr diesel it was without any dought the most reliable vehicle I have ever had the pleasure to own, sadly rust brought about its departure, I was very sad to say goodbye to him, yes him. My Corolla's demise brough about yet another Toyota a lovely RAV4 2ltr Diesel again, the Rav hasn't been without its issues, but has always done what's been asked of it. The most recent hiccup has been a severe shortage of power, occasionally very little power down 27mph & the engine fading severely to the point of displaying engine fault lights. I could go on, but after several days I replaced the feul filter, & bingo the problems disappeared completely, now the Rav4 diesel accelerates cleanly is much livelier in all aspects & feels completely different to drive. I'm just waiting now for the fault lights to repair themselves & my Fav will be a happy bunny again.
