D4D Fuel Pump cut off

Hi Guys, 

Your superior knowledge and expertise would be gratefully recieved. 

I have a 2003 AVENSIS D4D MPV 7 seater, which starts and runs fine. It revs fine, Sounds good. No warning lights up,

When I put it into gear and move off, (even when cold) it just dies. I have already tried disconnecting the MAF sensor, and it still does the same. From the way it cuts out I am thinking it may be the fuel pump cut off. 

1) Any ideas on possible causes?

2) Where is the fuel pump cut off located and is it possible to bypass it to prove if it is this?

 

I did have a good look around the site trying to find a similar thread first before posting.

Further information. If you start it normally it runs fine. Into gear and off you go and then it cuts out

If you start it and hold the key in the start position it continues to drive normally. As soon as you release the key from the start position it will cut out if you are in gear.

you might want to think about replacing the ignition key switch - pretty sure that this has caused a few people issues over the years. e.g.
