E111 Form

Hi guys,

I need a bit of help filling in a form for The Ring Trip ..

I have no Idea what the following are:

1.Personal Identification Number

2.Name of Institution

3.Identification Number of the Institution

E111 Form

Can any one help?

i think that Personal Identification Number is your national insurance number!

but i could be wrong

Nah man, haven't tried the postie.

I saw that at the top, but as it was called the E111 ... i just presumed it was right!lol never had to fill one of these out you see!!!

This is the site it was off, it says you just hand it in to the post office and then they stamp it and return it to you .. im confused!

i got mine from post office! gave me all the info i needed and talked me through some of the bits

It doesn't look like it mate ..

Basically, Ken said that you should only have to fill out:

1.Last name

2.First name(s)

3.D.O.B

4.National Insurance Number

That's it.

Me, Alan_21, and Bibbs are all having to go into postie town tomorrow and do them at the post office!!

It should just be a straight and simple few self explanatory questions.

Hanging out with a few guys that are going tonight to have a few final bevvies before hand .. I also have tomorrow off work so I am going to finish up a few things and just prepare myself for the absolute mayhem ahead! :P

Back home now and just checked mine, actually they are both correct yours and Chappers.

The one Chappers has posted is the one the post office keep's and the one you've posted Dave is the one you take with you along with the booklet telling you who to contact in each country in the event of illness

Still need the booklet though so best going along to the post office ;)
