P0201 code when diesel around 25% of fuel tank

Hello everyone

For the third time, when fuel is about at 25% of the tank capacity, the care get a tiny stutter and I get a p0201 on the scanner.

Tried checking the fuel cap, seems ok. Even took it off at full tank, no code. Tried toyota additive to clear injectors. Suggestions?

P0201 = Injector Circuit / Open - (Cylinder 1)
