Vacuum Hoses

As all will soon discover, I am Automotively challenged and I need help.

Some of the small (1/8" ID) Vacuum hoses between the Fuel injection system and the small cooling water manifold on the block have Dry rot and fallen apart. I cannot figure out which ones connect to which. Also, a hose that connects between two sensors? right behind the Valve cover, blows off after a few minutes of driving.

Do I have a faulty part causing excess pressure? Can someone provide a diagram for the 4 other hoses? Thanks, Terry
